What should I bring to my appointment?

The list below seems like a lot, but the more information you can provide about your current and previous health conditions helps to provide a more comprehensive and informed health assessment.

  • A Valid Referral letter from your GP (General Practitioner) or specialist. (In order to receive a Medicare rebate, you will need to have a valid referral letter. A referral from your GP is typically valid for 12 months but your GP can also make it ‘indefinite’. A referral from another specialist is only valid for 3 months)
  • Your Medicare Card and a valid ID (on your first appointment)
  • The name and contact details of other health professionals you currently see
  • Your current medications– the list of all your current medications (or the packages/ Webster-pak® medication pack)
  • A list of any supplements that you take (ideally bring them along to the appointment)
  • Diaries of records (Such as your documented weights; blood glucose or home blood pressure recordings; symptoms)
  • Results of any recent tests: blood tests/urine tests/ Xrays/ scans
  • For people with Diabetes: Home glucose monitors and your recent blood glucose records
  • For VicRoads Medical Reports, people with diabetes should attend with a record of their blood glucose levels and results of a recent optometrist report and have had a HbA1c blood test obtained within the previous 3 months.

What should I expect at my appointment?

When you attend your appointment with us, you can expect a comprehensive assessment:

  • We will have a detailed and focused discussion about you and your health.
    • We will go into some detail about your current symptoms, medications, treatments, medical history, and your concerns.
    • Depending on your circumstances the evaluation may include completing a questionnaire.
  • There will likely be a careful targeted physical examination
  • We will explain any further tests that are recommended or treatment options that we propose, taking your preferences into account.
  • We will put together an assessment and a plan for your management.
  • Any questions you have will be welcomed.
  • We will work with your other healthcare providers.

Fees

Fees will reflect the complexity of your health matters and the length of consultation.

Consultations are eligible for a Medicare rebate with a valid and in date referral that should be sent prior to your appointment or brought with you at the time of your consultation. The Medicare rebate can be processed at the time of payment if your bank account details have been registered with Medicare.

Payment is required in full on the day of consultation. We accept most credit cards (except American Express) and EFTPOS payments.

Private health insurance does not cover outpatient medical consultations.

  • How do I make an appointment?

    You can use one of the following options to make an appointment:

  • Will I receive confirmation of my appointment?

    Once you have contacted us and have scheduled an appointment, you will be posted a confirmation letter. The letter will detail the date and time of your appointment, consultation fees and practice location.
    In addition, prior to all appointments a confirmation text will be sent to your mobile phone.
  • How do I pay for my appointment?

    Payment is required in full, on the day of consultation. Payment can be made by Eftpos or credit card (American Express is not accepted).

    If you have already registered your banking details with Medicare then we can electronically lodge your rebate claim on the day of your consultation.

  • How can I reschedule or cancel an appointment?

    If you wish to reschedule an appointment please contact our reception staff directly on (03) 8548 0325. Cancellations and rescheduling requires at least 24 hours’ notice otherwise a non-Medicare rebatable cancellation fee will be charged.

    Arriving by car: There is limited parking available on site and more on- street parking on surrounding streets.
    Arriving by Public Transport: We are located just a short walk away from Essendon train station. You can also catch bus routes 476 or 477 or tram 59 (stop 41 at the corner of Fletcher and Napier Street).

  • Do I need a referral to see a specialist?

    Yes, a referral is required to make and appointment with us. Please make sure this has been organized prior to your appointment.

  • Fees
